Community Support Specialist
Under the direction of the Program Director, the Community Support Specialist will enhance the quality of client concerns, enhance satisfaction and promote quality and cost-effective interventions and outcomes for all clients as assigned. Community Support Specialist duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human service needs.
Specific Responsibilities/Duties
- Coordinate, provide and document client-centered care that is timely and effective.
- Handle case assignments, draft service plans, review case progress and provide feedback for case closure.
- Develop effective working relations and build necessary supports throughout the entire case management process.
- Adhere to professional standards as outlined by protocols, laws, rules and regulations.
- Evaluate clients to determine care and action plans and assist with treatment plans.
- Complete expanded assessment of client at initial meeting and continually assess and address motivational and psychosocial issues.
- Facilitate investigations, medical and treatment referrals, and ongoing legal proceedings.
- Works with agencies and service providers to facilitate case coordination and information sharing.
- Provide education and literature about diagnosis to help clients understand their condition(s) and treatment options.
- Interpret medical documents as needed.
- Evaluate client satisfaction and quality of care provided.
- Assess client’s progress throughout affiliation with Urban Rehab (UR).
- Refer cases where client and/or family would benefit from counseling to UR therapist.
- Review treatment goals and resources.
- Participate in the development and implementation of client care policies and protocols.
- Maintain proper client and program participant records, ensuring they contain current information and remain well-organized and categorized.
- Connect clients with necessary supports i.e., provide information and assistance to individuals interested in emergency assistance, housing, financial help and other forms of public help (this might involve making home visits and attending group meetings).
- Accompany clients to medical appointments and community activities when necessary to ensure their needs are properly met.
- Interview potential clients and their family members, and use the information to compile a file related to their drug, social, financial, educational and criminal background and help client and family make informed decisions.
- Follow-up on appointments, community activities and forms to ensure they’re properly completed.
- Assist clients in developing his/her treatment plans and offer clients advice and information regarding daily life care and services, food stamps, proper financial management, housekeeping and cooking skills, and becoming and remaining self-reliant.
- Work with supervisors, managers and other necessary department personnel to develop, review and refine current programs for families, single parents, mental health clients and other individuals in need.
