Now we are hiring for an SHELTER CASE MANAGER position.
Job Summary
The Shelter Case Manager plays a vital role in supporting individuals and families experiencing homelessness or housing instability. This position involves assessing client needs, developing personalized service plans, and connecting clients with essential resources to promote stability and self-sufficiency. The ideal candidate will demonstrate compassion, strong communication skills, and a commitment to empowering clients through effective case management.
Qualifications:
- Bachelor's degree in Social Services or related field; or Associate degree with 4 years of Case Management experience working with economically disadvantaged populations
- Strong written and verbal skills
- Bilingual Spanish fluency (desirable)
- Excellent communication and multicultural sensitivity skills
- Proficient in Microsoft suite and internet search
- Valid Driver's license
Responsibilities:
- Conduct CARES Assessments and create Independent Living Plans within 48 hours of client intake
- Identify client needs, develop tasks, and provide weekly/bi-weekly progress notes
- Coordinate medical, mental, and social service appointments
- Assist clients with social services cases (e.g., Public Assistance, Social Security)
- Provide referrals to educational institutions, job training/placement agencies, and other resources
- Attend case conferences, training sessions, and workshops
- Follow agency policies, procedures, and funding source guidelines
- Perform daily/weekly unit inspections and escort clients to housing referrals as needed
