Position Responsibilities:
The Chronic Care Coordinator works closely with the physicians and mid-level providers in a team setting to deliver high quality patient care in an efficient and effective manner. This position requires an energetic person with great interpersonal skills and the ability to work efficiently and effectively in a fast paced, ever changing environment.
The Chronic Care Coordinator is responsible for providing concierge quality medical care to patients with complex chronic illnesses. The Coordinator will complete this task by performing the following:
- Help the physicians and medical staff find and enroll new patients to the Chronic Care Program who have chronic illness that will benefit from constant monitoring
- Monitor care and health status monthly for all assigned patients ensuring patients are being compliant with their medical plan and obtaining the needed appointments and diagnostic studies.
- Perform status checks via phone to these patients to ensure the care plan is being carried out and all issues are addressed.
- When patients report issues or abnormalities in the care plan, immediately work with the appropriate provider nurse to quickly and efficiently solve these issues and ensure quality patient care

Minimum Skill Qualifications:
- MA license preferred, but not required
- Three (3) plus years of experience in the medical field required
- Three (3) plus years of relevant job experience required
- Ability to work well in a team setting
- Exceptional written and verbal communication skills
- Strong work ethic and the ability to learn in a fast-moving environment
Technical Requirements:
- Ability to use Microsoft Outlook
- Ability to use Microsoft Word and Microsoft Excel
- Experience with Allscripts is a plus
