- Conduct comprehensive assessments for nursing home residents.
- Develop and implement effective care plans in collaboration with multidisciplinary teams.
- Monitor patient progress and adjust care plans as needed.
- Maintain accurate documentation in compliance with regulatory standards.
- Participate in quality improvement initiatives to enhance clinical services.
- Licensure & Certification:
- Active Nurse Practitioner license and Board Certification in Indiana.
- Active NPI (National Provider Identifier) and DEA (Drug Enforcement Administration) License.
- Active Medicare Provider Number in Indiana.
- Experience:
- Minimum of 6 months practicing as a Nurse Practitioner.
- Preferably at least 6 months in a Skilled Nursing Facility or Long-Term Care setting.
- Additional Requirements:
- Valid Indiana Driver’s License and proof of automobile insurance.
- Ability to work within the specified hours, Monday to Friday.
