- Do in-depth assessments and manage chronic conditions
- Close care gaps (HEDIS and STAR)
- Lead care planning with a full clinical team

- Unrestricted APN license and board certification
- Eligible to treat Medicare patients
- 6 months in a skilled nursing or long-term care setting
- Comfortable with digital tools and team communication
- $15,000 Clinical Quality Bonus
