What You’ll Do
- Assess nursing home residents.
- Build care plans. Work with facility teams.
- Track patient progress. Adjust care when needed.
- Keep clean, accurate records.
- Help improve clinical quality.
- Active NP license and board-certified in Texas.
- NPI and DEA licenses.
- Active Medicare provider number (Texas).
- At least 6 months of experience as an NP.
- Preferably 6 months in SNF or LTC settings.
- Valid TX driver’s license insurance.
