Here’s What You’ll Actually Do:

• Manage your own panel of patients

• Perform in-depth assessments and manage chronic conditions

• Close care gaps (HEDIS and STAR measures)

• Lead care planning with a full clinical team

• Help patients and families feel confident in their care plan

You’ll Need:

• Active, unrestricted APN license board certification

• Eligible to treat Medicare patients

• 6 months in skilled nursing or long-term care

• Comfortable using digital tools and working with a team
