Physical Therapist (PT) | $2,500 Sign-On Bonus | Full-Time & Part-Time Opportunities
The Role
You're a licensed Physical Therapist who knows how to help patients recover, stay safe, and move better at home. You'll visit patients where they live and deliver personalized care plans that focus on mobility, strength, and quality of life.
We need someone who can work independently, document clearly, and communicate well with both families and providers. Full-time is ideal, but part-time therapists are welcome to apply.
What You’ll Do

Do in-home evaluations and regular assessments (including OASIS)

Build and update care plans with physician input

Treat patients using exercise, equipment, and other physical methods

Teach families how to support the therapy plan

Supervise PTAs and paraprofessionals when needed

Keep clear, timely notes and records
-
Coordinate care with nurses, home health aides, and other team members

Attend case conferences and staff meetings
What You Need
-
Licensed PT in Virginia
-
1 year of experience (home health preferred)
-
CPR certified
-
Valid driver’s license and reliable transportation
-
Able to pass background checks and employment verification
-
Familiar with Medicare guidelines (a plus)
