Your Mission as a Social Worker
As a Social Worker - Contract Nursing Home Program Coordinator, you will be at the forefront of transforming the care experience for Veterans placed in nursing homes. You’ll take charge of coordinating personalized care plans that ensure every Veteran receives compassionate, top-tier support tailored to their unique needs. Your role will empower you to:
- Care Advocate: Lead the management and oversight of Veterans’ care in contract nursing homes, becoming a trusted advocate who champions their well-being every step of the way.
- Collaborative Innovator: Drive innovative solutions and improvements by collaborating with healthcare teams and community partners, ensuring Veterans benefit from the best resources and services available.
- Veteran Liaison: Act as the vital link between Veterans, their families, and the VA, providing guidance, reassurance, and support that makes a real difference in their lives.
- Quality Enhancer: Monitor and elevate the quality of care in nursing homes through proactive problem-solving and continuous program enhancements.
- Policy Developer: Shape the future of the CNH program by developing and implementing policies that uphold the highest standards of care and compliance.

Social Work Qualifications and Experience
- Master’s degree in social work from an accredited institution.
- Current, unrestricted license in social work.
- Experience in case management and care coordination, preferably in a long-term care setting.
- Knowledge of VA policies and procedures related to contract nursing homes.
Social Worker Salary and Benefits
- Social Worker Salary: Annual range of $96,139 - $124,986 per year
- Social Work Salary: Hourly range of $46.22 - $60.09 per hour
- Annual Salary Increases: 2% federal, 2% local, and promotional (on average)
- Retirement: Thrift Savings Plan (TSP) retirement (401k) with employer contributions
- Pension: Federal Employees Retirement System (FERS) pension contributions
