The Patient Navigator is responsible for supporting a clinical team of physicians and nurses by acting as a liaison between the patient and their care team. The Patient Navigator guides patients through all aspects of pre-treatment requirements, manages patient expectations, and assists them in all aspects of helping them to begin their treatment plan. Also provides assistance in a variety of administrative roles to the clinical team for each patient undergoing treatment.

We have an immediate opening for a full-time Patient Navigator to join our Norfolk, VA location. The schedule is Monday-Friday 7:30am-4:00pm, with the potential for weekend and/or holiday hours as needed.

How You'll Contribute:

We always do whatever it takes, even if it isn’t specifically our “job.” In general, the Patient Care Coordinator is responsible for:

Patient Interaction:

  • Meets with established patients and assists with coordination of pretreatment requirements in conjunction with physician orders
  • Provides patient education regarding clarification of physician instructions, medication instruction, and anticipatory guidance under the supervision of the RN
  • Establishes a compassionate environment by providing excellent customer service, anticipating anxieties, answering questions, providing support
  • Assures quality care by adhering to Practice standards; follows clinical operations policies and procedures, as well as standards of care set by the state and other governing agency regulations

Clinical/Technical Duties:

  • Documents patient care services by charting in patient and department records including the electronic medical record (EMR)
  • Utilizes the computer in entering labs and printing labels
  • Maintains safe and clean working environment by complying with procedures, rules, and regulations, and calling for assistance from other health care support if needed
  • Authorized to receive and disseminate lab results from outside laboratories to the patient’s RN and/or physician for review and follow up as necessary
  • Assists the primary clinical teams with daily results as needed. Communicates results to patients via call backs under the supervision of the RN
  • Utilizes the Patient Care Module in the EMR to provide consistent and timely patient follow up and management of testing and cycles

Administrative:  

  • Initiates patient charts and sets up check lists in the EMR
  • Scans forms, records, and other information as needed into the EMR
  • Responsible for generating and maintaining patient pretreatment requirements in conjunction with clinical protocols
  • Communicates with financial coordinator to ensure authorizations obtained prior to starting treatment. Assists with completing and routing needed insurance forms (i.e., insurance questionnaires).  Faxes test results to financial coordinator and/or insurance plan as needed
  • Utilizes scheduling system for purposes of managing the primary physician’s schedule and scheduling patients for the primary physician
  • Communicates problems to the primary clinical team, i.e. pretreatment or financial concerns
  • Reviews patient records for completion of prescreening inclusive of medical history, appropriate laboratory work, and informed consent
  • Checks that consents are current and entered into the EMR. Informs patient/couple of consents needing to be updated
  • Assists in obtaining authorizations for medications. Communicates with insurance plans and pharmacies to ensure patient able to obtain medications in timely manner
  • Documents laboratory results in the EMR checklists after review the by RN and/or physician
  • Documents delivery data in the EMR. Reviews lists for team’s patients needing delivery data entry and notifies RN/assists RN with collection
  • Prepares patient information packets as needed
  • Assists team with correspondence and other administrative tasks as needed
  • Contributes to the upkeep of office equipment and daily “operational” functions within the office. (i.e. fax, printer, mail, filing, coping)

Team Communication and Interaction:

  • Assists the clinical team to resolve patient problems and needs by utilizing multidisciplinary team strategies
  • Communicates with other offices if patient to be monitored or have procedure there to ensure accurate information available and needed testing entered as an order in the EMR
  • Assists in patient retention activities through utilization of the Patient Care Module in the EMR to continually improve services and productivity, increase patient retention, and enhance patient satisfaction
  • Reliable - reports to work on time, as scheduled, ready to work on a consistent basis
  • Covers weekends and holidays as assigned
  • Other duties, as assigned:

               1. All Patient Navigators:

        • Assists front desk staff with answering phones, checking patients/in & out, and other administrative duties
        • Assists with monitoring as needed: sets up room for procedures; cleans and stocks procedure rooms and blood draw station; cleans and cares for equipment according to manufacturer and OSHA guidelines; assists in maintaining patient medical records through the use of the electronic medical record

                   2. Patient Navigators with PCC, MA, LPN, medical, or other appropriate training:

          • Assists the physician by performing phlebotomies, taking vital signs/ weight/height, assists provider during examinations and procedures (ex: IUIs, endometrial biopsies, ultrasounds, etc.), preparing samples and specimens for procedures as needed, and completing accompanying paperwork as necessary
          • Ensures lab specimens are sent to proper labs
          • Teaches injection classes (OI/IUI) as needed

      What You'll Bring:

      • High school diploma required; 1 year experience in medical or healthcare setting (women’s health experience a plus)
      • Medical or clinical training (MA, LPN) preferred. If LPN, current state LPN license without limitations required
      • Patient Navigator with MA or LPN responsibilities: Current CPR certification or CPR Certification acquired within 1 month of employment
      • Ability to access, input, and retrieve information from a computer
      • Knowledge of office procedure and office machines (i.e. computer, fax, copier, etc.)
      • Ability to demonstrate good judgment, critical thinking, and problem solving abilities
      • Ability to function as a member of a multi-disciplinary healthcare team
      • Ability to communicate accurately and concisely
      • Excellent interpersonal skills and patient orientation
      • Ability to work independently to manage a caseload of patients
