Duration : 3 months contract (with possible extension) Job Description : Note : This is a remote position, but frequent travel may be required to visit members at home, hospitals, skilled nursing facilities, or physician offices. We are seeking an experienced Registered Nurse Case Manager (RNCM) to join our Senior Care Options (SCO) team. In this role, you will manage the care of frail elderly members, working closely with physicians, caregivers, and community resources to ensure a comprehensive, patient-centered approach.Key Responsibilities : Conduct initial and ongoing geriatric assessmentsDevelop and communicate individualized care plansComplete Minimum Data Set-Home Care (MDS-HC) assessmentsCoordinate community resources such as Adult Day Health, Adult Foster Care, and Personal Care Attendant ProgramsParticipate in primary care team meetingsManage transitions of care across various healthcare settings (ED, hospital, rehab, nursing home, home care)Serve as an advocate and facilitator to resolve barriers to careProvide education and coaching to members and caregivers on health status, treatment options, and benefitsQualifications : Skills : Strong ability to work autonomously and prioritize workloadExcellent team collaboration and problem-solving skillsExperience navigating community resources and care coordinationCultural competency and sensitivity to diverse populations Education : BSN degree and Registered Nurse (RN) license (unrestricted, active)Experience : 3 years of clinical nursing experienceCase management experience managing geriatric / chronic illness populationsExperience in Senior Care Options (SCO) programs preferredKnowledge of Medicare / Medicaid managed care preferredProficient in health information technology and computer systemsCase Management Certification (CCM) a plus About US Tech Solutions : US Tech Solutions is a global staff augmentation firm providing a wide range of talent on-demand and total workforce solutions.
