Responsibilities
Abides by and enforces all compliance requirements; adhere to hospital policies including confidentiality; exhibits proficiency when utilizing and accessing both the HIS (Hospital Information System) and CARLOS; informs supervisor when supplies are low; maintains clean work environment; processes incoming transcription reports at a minimum of 12 transcribed reports per hour; maintains daily logbooks regarding transcribed reports; processes incoming loose reports; maintains a neat and orderly method for storing loose reports until they can be posted on the charts at a minimum of 30 loose reports per hour; assists physicians, Business Office and other medical record requestors by pulling charts, answering questions and performing follow-up regarding chart completion requirements; maintains the Incomplete File area, which includes checking the booths and filing charts from the return basket; performs weekly review of CARLOS Report SR0004 to identify and concentrate on physicians with an excessive number of incomplete/delinquent medical records; performs weekly review of CARLOS Report SR0013 to identify and concentrate on those charts that are delinquent for the assigned areas; identifies and pulls charts for the various Grand Rounds; assists with the assembly and analysis of records as needed; conducts reanalysis of incomplete medical records at a minimum of 15 inpatient or 20 OP Surgery/Observation records per hour; uses appropriate customer relations skills with physicians, patients, families and healthcare team in person and via telephone; responds to overhead pages; informs/relays information to incoming shifts regarding any unfinished requests/duties; assists persons seeking medical record information by having requesters fill out the appropriate forms; assists persons needing records from the archive areas; files all records and microfilm demonstrating a 99% accuracy rate; maintains adequate file space by shifting records when necessary; assists in boxing records for off-site microfilming; reviews requests with supervisor daily for work assignment; retrieves all medial records requests during the day/evening weekend/shifts for patient care; properly out-guides all records for internal and external requesters; maintains emergency room records, attaches all loose test results and files accordingly; follows-up with supervisor on all records not retrieved within 72 hours; identifies and posts all forms and reports in the correct chart order; completes daily department routing slip; maintains accurate logbook; updates CARLOS as needed; removes specified forms/reports from the medical record for reviewers/requestors; verifies information sent via fax was done without error; adheres to current Infection Control and Safety regulations; regular and prompt attendance and the ability to work schedule as defined and overtime as required; related duties as required.
Qualifications
High school diploma or equivalent and one year of general clerical experience in medical records.
