Responsibilities
The Patient Safety Observer (CW) ensures the safety, health, and comfort for patients experiencing mental or behavioral crisis or pose a risk to their own safety because of developmental, medical, or cognitive status; create a safe and therapeutic environment through constant observation and interaction with the patient; the primary role is safety, but do not perform critical care; may be assigned, but not limited to, the following patients: suicidal ideation or attempt, mental or behavioral crisis (schizophrenia, bipolar, depression, mania), homicidal or threats to others, elopement or wander risks, fall risk, confused, mental or cognitive impairment (some types of autism), and any other condition or behavior the medical or nursing team deem necessary to ensure the safety of the patient (pulling at IV lines, tubes, or drains); knowledgeable and adaptable to understand and respond to the emotional states and mindsets of individual patients; top applicants are observant, respectful, diplomatic, and compassionate; knowledge or experience with age specific development and/or mental health conditions; supervise assigned patients at all times, keeping constant visual observation of the patient at a safe distance; this includes remaining with the patient at all times, unless otherwise communicated by the nursing or child life team, accompany the patient off the unit, ensure the patient is never out of sight to include while in the restroom; supports and assists with activities of daily living, which can include showering, toileting, diversionary activities, meals, feeding, and inspects tray to ensure any potential risk items are removed prior to giving to the patient; notifies nursing staff when patient help, escalating behavior or the patient indicates worsening of feelings is identified; maintain confidentiality of patient information; adhere to relevant policies, procedures, and standards of care; engages in therapeutic conversation and listening to the patient and escalates concerns to the primary nurse; ensures that conversation remains within therapeutic boundaries and does not convey bias or stigma; participates with the care team in the environmental assessment and can remove objects if they are a concern for patient safety; monitor visitor and patient belongings to comply with safety; maintains professional communication with the patient at all times; communicates to the patient and family of the role and responsibilities; receives hand off from the care team to understand what behaviors to observe for and risks; understand the observation levels and adheres to the standards of the assigned roles (constant observation, 1:1, 2:1); uses de-escalation techniques in daily practice and during periods of increased agitation to help mitigate the situation; records observations in the patient record; demonstrates age specific and cultural competencies for the population serves to include family and visitors; values cultural diversity and individual differences in the workforce and patient population; performs other related and delegated responsibilities as assigned and refrains from functions not deemed competent; outstanding communication, listening, and observation skills; basic knowledge and understanding of medical terminology, understanding of the healthcare environment, and functional knowledge of infection control strategies; ability to work as part of a team and display a calm, confident manner that is not deterred by combative or distressed patients; regular and prompt attendance; ability to work schedule as defined and overtime as required; related duties as required.
