Job Purpose: The Case Manager is a participant in the collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs. This process includes medical, social, developmental, behavioral, financial, and educational intervention with focus on quality and cost-effective outcomes.
Education & Qualifications: Nursing ADN, ASN, BSN, or MSW preferred. Possesses Case Management Certification or will obtain CCM Certification within first year of employment.
Required Licenses/Certifications: RN License
Work Experience: Three to five years of acute care hospital experience preferred. Critical care or emergency department experience highly desirable. Demonstrated skills in the areas of negotiation, communication (verbal and written), conflict, interdisciplinary collaboration, management, creative problem solving, and critical thinking. Knowledge of healthcare financing, community and organizational resources, patient care processes, and data analysis. Knowledge of utilization management as it relates to third-party payers. Knowledge of post-acute care community resources. Experience with managed care preferred. Excellent verbal and written communication skills required. Demonstrates flexibility via an ability to adapt to changing priorities and regulations. Basic computer skills required. Ability to work in a fast-paced setting, meeting timeline.
Physical Demands & Working Conditions: Mostly sedentary, lifts up to 10 lbs., walks, stands may climb stairs. Work is light with some reaching, handling, and fingering to operate office machines. Ability to work with spreadsheets, multiple computer programs, and dual screens.
Special Requirements: Verbal and writing ability required to communicate effectively in informing employees of activities and policies. Numerical ability required. Ability to receive, comprehend, and carry out instructions; ability to work safely with equipment, supplies, and materials; ability to comply with attendance requirements; ability to comply with Human Resource policies. Ability to communicate on various levels to include management, client, and customer. Strong supervisory, leadership, management, and coaching skills.
Essential Duties and Responsibilities:
- An autonomous role that coordinates, negotiates, procures services and resources for, and manages the care of moderate to complex patients to facilitate achievement of quality and cost efficient patient outcomes.
- Looks for opportunities to reduce cost while ensuring the highest quality of care is maintained. Works collaboratively with inter-disciplinary staff internal and external to the organization.
