Job Description
Under general supervision, responsible for receiving, reviewing, processing, and tracking requests for patient medical records.
Responsibilities
Serves as a HIM Ambassador and Patient Liaison by ensuring rights to health information including privacy and right of access while encouraging patient engagement in healthcare by providing easy and timey access.
Serves as subject matter expert by providing guidance on privacy and disclosure matters.
Monitors and manages multiple sources for incoming requests for information such as patient portal, fax, mail, e-mail, and in person.
Reviews release forms to ensure state/federal rules and regulations are met prior to any release of health information.
Answers calls received from patients and other requesters regarding requests for health information and status updates.
Performs data entry for submitted request forms for health information into the Epic Release of Information (ROI) Module.
Reviews, prepares, and scans ROI documents for inclusion in EpicCare - the Electronic Medical Record (EMR).
Researches and identifies the correct patient health information to be released in accordance with each request and according to federal and state rules and regulations.
Locates patient health information from multiple source systems (Epic, Mosaiq, IDX, Piers, Harmony, etc.), locations, and formats (paper and electronic).
Copies/prints/uploads health information from the Designated Record Set; processes billing for copies when applicable.
Manages and prioritizes Release of Information requests into work queues based on type of request (patient, continuing care, insurance, disability, legal, workers comp, etc.) to maintain the state regulated turnaround time of 15 calendar days.
Protects the privacy of patient information in accordance with HIPAA/State/Federal rules and regulations.
Qualifications
Performs all other duties as assigned.
Working knowledge of State and Federal rules and regulations governing the release of health information inclusive of HIPAA and the Privacy Rule.
Working knowledge in more restrictive privacy and security regulations regarding the release of substance abuse, psychiatry, and genetic medical records.
Knowledge of medical terminology, clinical documents, and electronic records applications.
Strong customer service acumen and interpersonal skills to effectively work and communicate with both internal and external customers and respond to requests in a timely and respectful manner.
Ability to maintain strict protocols of all confidential or sensitive information.
Strong attention to detail and sound judgement required to ensure accuracy of the legal medical record.
Proficient computer skills using Microsoft Office, database programs, and the Electronic Medical Record.
Ability to work independently and meet turn around time and productivity standards.
Demonstrates the ability to support the mission, vision and patient promise throughout all interactions, the UT Health Way.
Experience
One (1) year of experience in medical records or relevant clerical experience is required.
