JOB SUMMARY
Join our team at the Ambulatory Clinic Endocrine & Weight Wellness! We are seeking a dedicated Nurse Diabetes Educator to conduct patient assessments, prioritize care needs, and involve family members in the education process. You'll develop and implement personalized educational plans, ensuring all documentation meets ADA and ADCES standards.
Key Responsibilities :
- Provide education in one-on-one sessions and group classes.
- Facilitate group discussions and contribute to program accreditation.
- Collaborate with the care team and involve supervisors for senior-level issues.
- Participate in quality improvement activities and health system initiatives.
- Develop educational materials and engage in community health events.
- Manage daily assignments and adapt to scheduling changes.

Works under general supervision to provide Diabetes Self-Management Education and Support (DSMES) for patients within a designated department or specialty area.

EXPERIENCE AND EDUCATION
Required
Bachelor's Degree in related field. Graduate of an approved / accredited nursing program.
Specialty areas may have increased experience requirements as dictated by clinic or standard of care.
RN) REGISTERED NURSE Current State of Texas nursing license to practice as a Registered Nurse Upon Hire
BLS) BASIC LIFE SUPPORT certification, course accredited by the American Heart Association (AHA) or American Red Cross (ARC) Upon Hire
Must hold and maintain Certified Diabetes Care and Education Specialist (CDCES) within 1 Year or
BC-ADM) Adv Diabetes Mgmt for CNS & NP within 1 Year
Preferred
1 year of clinical nursing experience
