Social Work Case Manager Position
Utah Case Management is looking for Social Work Case Managers to join our team. Social Work Case Managers coordinate services for our elderly and mental health clients through Utah Medicaid's New Choices Waiver program. The position is part-time based on your needs and the needs of the company. We offer the benefit of a flexible work schedule allowing you to govern your own hours and schedule. This is a hybrid position with remote documentation and in person client visits. We are looking for dedicated employees to coordinate services for our clients. We are currently looking for someone in Washington County. See below for responsibilities and qualifications:
Responsibilities:
- Managing clients' services: monthly face-to-face contact with clients, assessing clients needs, coordinating services for clients, communicating with service providers, etc.
- Documentation: completing, updating, submitting, and compiling necessary documentation for clients.
- Regularly meeting and communicating with clients at their home/assisted living facility/long-term care facility
- Communicating with other employees to coordinate clients' services
- Regularly attending staff meetings and company trainings
- Able to use your own devices for this position
- Able to travel to visit clients each month
Qualifications:
- Utah Resident and residing in the south part of Utah County
- Current SSW license, CSW license, LCSW license, or RN License in the State of Utah
- Good at building and maintaining relationships with clients, clients families, and facilities
- Detail oriented
- Self motivated
- Independent and team worker
- Good communication and organizational skills
Pay Rate:
- Billable rate $27 (billable rate is anything that can be billed to Medicaid for the client)
- Non-billable rate $15 (non-billable is related to the position, but not client related, i.e. training, team meetings)
Job Types: Full-time, Part-time
Pay: $15.00 - $27.00 per hour
