Responsibilities:
- Assess and monitor patients’ conditions to ensure the best possible care.
- Conduct home assessments and provide patient care as required.
- Ensure compliance with state regulations and document care accurately.
- Use the latest technology and practices to evaluate and treat patients.
- Collaborate with the care team and provide valuable input on patient treatment plans.

Qualifications:
- Licensed as a Registered Nurse (RN) by the State of Utah.
- Home Health or Hospice experience is preferred.
- Experience with Homecare Homebase or other EMR systems is a plus.
- Strong verbal and written communication skills.
- Ability to maintain patient confidentiality and privacy.
- Valid Utah driver’s license and automobile liability insurance.
- CPR certification.
