Job Overview:
Utah Home Health and Hospice is looking for a dynamic, motivated, and compassionate RN to join our patient -centered care team. The primary responsibility of the home health and hospice RN is to provide direct support to the patient and their families while functioning within a medically approved plan of care. The ideal candidate will be a responsible and well-trained professional able to give the best nursing care with little supervision. You will be compassionate and cool-tempered.

Principle Duties and Responsibilities:
* Work as part of an interdisciplinary team that includes physicians, nurses, social workers, chaplains, massage therapists, certified nurses aides, etc.
- Perform on-call responsibilities as needed by the organization.
- Work with primary care physicians and the hospice medical director.
- Accept and understand the mission of our home health and hospice.
- Ensure that the educational needs of the patient and family are met.
- Review and document patient needs in care plans, progress notes, etc.
- Monitor patient’s condition and assess their needs to provide the best possible care and advice
- Administer medications and change wound dressings
- Provide instant medical care in emergencies
- On-Call: 7 on 7 off
- Travel required
Qualifications:
- Must possess current, unencumbered, active license to practice as a Registered Nurse.
- Must have valid care insurance and driver's license.
- Knowledge of and experience in pain and symptom management.
- Well-developed physical assessment skills.
- Must be able to read, write, speak, and understand the English Language.
- Must possess patience, tact and a positive attitude.
- Hospice/Home Health experience preferred.
- Must have valid CPR.
