- Assess, supervise and direct patient care.
- Perform patient care as required, including home assessments.
- Develop plans of treatment.
- Ensure and chart compliance with state regulations.
- Evaluate and treat patients using the most current technology and practices.
- Provide valuable input into the patient's care and treatment.

Home Health and Hospice are both challenging and rewarding! This job requires a person who will never forget our work requires a team effort. Other requirements include:
- Licensed as a Registered Nurse by the State of Utah
- Minimum 2-3 years of Hospice Experience
- Experience with Homecare Homebase or other EMR not required but a HUGE plus
- Ability to communicate effectively, both verbally and in writing
- Respecting the patient’s privacy and maintaining confidentiality
- Current Utah driver’s license and automobile liability insurance
- CPR Certification
