As a Social Worker at Utah Home Health and Hospice, you are responsible for evaluation and treatment of Home Health and Hospice patients by developing a plan that addresses the patient's psycho-social status, physical status, and emotional needs. You will develop bonds with our clients, helping them work through life's biggest transitions. Job Description:
- Provides social work services to meet the emotional needs of the patients and family.
- Demonstrated ability to engage interdisciplinary team, staff, patient, family members.
- Goals of care discussion
- Hospice info visits
- Emotional support
- Other duties as assigned.
Job Requirements:
- *Home Health and/or Hospice experience preferred but not required
- Master's degree in social work. Education must be obtained from an accredited institution. Degree will be verified.
- Current State Licensure as a CSW or LCSW
- Must have an active DOPL license.
- Current Utah driver’s license and automobile liability insurance
- Basic computer skills and knowledge of word processing and spreadsheet software.
- Strong written and verbal communication skills.
- Demonstrated ability to exercise critical thinking skills.
- Must possess solid Clinical, Organizational, and Communication skills.
- Experience with Homecare Homebase or other EMR not required but a HUGE plus.
