JOB SUMMARY: The Hospital Pharmacist is responsible for the safe and effective use of medications within a hospital setting. This role involves reviewing prescriptions, ensuring appropriate medication therapy, providing drug-related education to healthcare staff and patients, and collaborating with the medical team to optimize patient care. The pharmacist ensures compliance with hospital policies and regulatory standards while maintaining accurate medication records and inventory. Reports to Pharmacy Director. Non- Exempt position.
KEY RESPONSIBILITIES:
Medication Management & Dispensing

- Monitor patient medication therapy for safety, efficacy, and potential adverse effects.
- Review and verify medication orders for accuracy, appropriateness, and potential drug interactions.
- Prepare and dispense medications in accordance with hospital protocols and regulatory guidelines.
Patient & Healthcare Team Collaboration:

- Provide drug information and education to physicians, nurses, and other healthcare professionals.
- Counsel patients on medication usage, potential side effects, and adherence strategies.
- Participate in multidisciplinary rounds to provide pharmacological expertise.
Regulatory Compliance & Safety:

- Oversee patient assessments, triage, and the implementation of nursing care plans.
- Ensure compliance with state and federal regulations, including Texas State Board of Pharmacy requirements.
- Maintain proper documentation and records of medication use.
- Conduct medication reconciliation to prevent errors and adverse drug events.
Inventory & Quality Control

- Manage and oversee pharmaceutical inventory, ensuring the availability of essential medications.
- Implement and follow quality assurance protocols to maintain high standards of pharmaceutical care.
- Participate in hospital initiatives for medication safety and process improvement.
Staff Development and Training

- Identify training needs and opportunities for professional development among the LVN and CMA staff.
- Coordinate and facilitate educational programs, workshops, and in-service training sessions.
- Stay updated with current nursing practices, research, and advancements in healthcare to guide staff members.
QUALIFICATIONS
1. Education and Certification
-
- Bachelor’s Degree and/or advanced degree in Pharmacy required.
- Current Texas Pharmacy license required.
2. Experience
-
- Minimum of 1-3 years of hospital experience in a healthcare setting preferred.
3. Skills and Competencies

- Strong knowledge of pharmacology and hospital protocols.
- Excellent communication and teamwork skills.
- Attention to detail and problem-solving abilities.
- Experience in clinical pharmacy or hospital settings preferred.
