Job Description
We are looking for an RN to fill the role of our MDS Coordinator. This role comes with a flexible schedule on the Day shift. The role could be filled by (5) 8-hour days or (4) 10-hour days, there is also no weekend requirement .
Relocation is available.
The sign-on bonus will be paid out in two installments : the first pay check and after completion of oneyearof satisfactory employment.
Please note that current University of Vermont Health Network employees are excluded from eligibility for this bonus.

Job Summary
The MDS (Minimum Data Set) ispart of a federally mandated process for clinical assessment of residents in Medicare or Medicaid certified nursing homes. The MDS Coordinator assumes responsibility for coordinating and completion of the MDS Form, Raps and trigger sheets and actively participates in the establishment, completion, and continued maintenance of resident directed care plans.Under the general direction of the Director of Nursing and in collaboration with the Unit Nurse Manager, they are accountable for insuring the completion of a comprehensive assessment of resident's needs and the development of a resident plan of care designed to meet resident needs.
Requirements
- Current RN licensure or compact licensure recognized by the State of Vermont required.
- Graduate of an approved school of nursing.
- Able to demonstrate leadership skills within facility.
- Able to read, write and speak the English language.
- Demonstrate competence as a nursing practitioner.
- Two or more years experience in long term care preferred .
- Experience with EPIC is preferred , but not required.
