POSITION SUMMARY
The Documentation Specialist monitors and maintains the timely receipt of documentation required for claims submission. This position is responsible for daily monitoring of documentation needed such as home infusion detailed written orders, patient consents, Advanced Beneficiary Notices (ABNs) and proof of delivery and follow-up to obtain needed documentation. The Documentation Specialist works with the Clinician Operations Manager to ensure documentation is obtained and scanned into the electronic medical record (EMR).
ESSENTIAL JOB KNOWLEDGE
- Monitors and records timely receipt of detailed written orders (DWOs) for home infusion therapy, patient consents, ABNs, and missing proof of delivery documentation for claims submission.
- Maintains a follow-up system for outstanding documentation such as reports and Work In Progress (WIP) states.
- Monitors daily reports for newly identified documentation needed. (E.g., consent log Brightree Ad Hoc Report
- Monitors WIP states for orders in unconfirmed state due to need for documentation.
- Follows up with any delinquent documentation. Reviewing outstanding documents at least weekly and contacting physician offices and patients as needed to get documents signed.
- Uploads compound reports to the patient profile within EMR.
- Prescription lot number entry.
- Learns and understands all aspects of the computer system, as it pertains to their position.
- Follows all Joint Commission regulations.
- Maintains patient confidentiality.
- Maintains a professional and courteous demeanor with customers, referrals, and co-workers.
- Provides cross coverage for Infusion Operations Assistant.
- Provides weekly updates to the supervisor on progress with assigned tasks and escalates any issues with outstanding or overdue documentation to supervisor.
- Other duties as assigned.
- Successfully completes initial and ongoing organizational and department education, training, and competency requirements.
- Infection Control: Understands, demonstrates, and follows organizational infection control guidelines (CDIC).
- Quality: Focuses on improving processes and champions efficiency in daily work activities.
- Safety: Promotes a culture of safety by taking accountability for individual actions, including but not limited to, promptly reporting errors or near misses via the organization’s occurrence reporting system and participating in developing improved safety processes, as appropriate. (ADM015, ADM016, OPS044, Mission Statement).

JOB REQUIREMENTS
Licenses:

Education:
- High school diploma or equivalent required.
Experience:
- A minimum of one year of recent experience in a healthcare position is required.
- Experience and strong knowledge base in home care billing experience preferred.
Miscellaneous:
- Ability to effectively communicate in oral and written form.
- Ability to establish and maintain effective working relationships with both internal and external customers.
- Ability to demonstrate efficient time management skills, including the ability to handle multiple tasks and prioritize effectively.
- Ability to be self-directed in accomplishing the coordination and responsibilities.
- Knowledge of and ability to use a variety of computer programs including Brightree, Microsoft Word, and Excel highly desirable.
- Knowledge of medical terminology and abbreviations.
