Work Schedule:
100% FTE, days. Standard schedule would be between the hours of 8:00 am - 5:00 pm Monday through Friday. You may be eligible for up to a $3000 sign-on bonus. No on call or weekend requirements. This is a hybrid position. When in person, you will work at 705 S. University Ave in Beaver Dam, WI.
Pay:
Working as an independent healthcare provider, you'll play an integral role in our care teams by providing high-quality, cost-effective care to patients while working collaboratively within a multidisciplinary health team. This position is part of the Collaborative Care workstream in Primary Care. As a member of the Collaborative Care team, you will work as a team with the primary care staff and team psychiatrist helping deliver brief interventions for patients in Primary Care.
We are seeking a Behavioral Health Therapist or Clinical Psychologist to:
Education:
Minimum – Master's Degree in Social Work, Counseling, Psychology, Marriage and Family Therapy, or other behavioral health related field
Work Experience:
Minimum – 2 years of experience providing evidence-based treatment for a variety of behavioral health diagnoses for children, adolescents, or adults
Preferred – 5 years of experience providing evidence-based treatment for a variety of behavioral health diagnoses for children, adolescents, or adults
Licenses and Certifications:
Education:
Minimum – Doctor of Philosophy (Ph.D.) or Psychology (Psy.D.) in Counseling or Clinical Psychology or related field.
Work Experience:
Minimum – One (1) Year Of Postdoctoral Supervised Clinical Experience

We are seeking a Behavioral Health Therapist or Clinical Psychologist to:
- Evaluate patients presenting with a wide range of mental health concerns and diagnoses.
- Collaborate with patients on individualized treatment plans with specific goals and outcomes.
- Coordinate patient care with primary care, other specialty providers, and families via written and verbal communication.
- Provide psychotherapy, utilizing multiple modalities and methods consistent with evidence-based practice, including specific and focused treatment protocols, when appropriate.
- Utilize progress monitoring and outcome measurement tools to assess benefits of psychotherapy and change the treatment plan, if necessary.
