Requirement description :

  • Must notate all during name clears
  • Board Certified (Addiction Medicine) REQUIRED
  • Board Eligible (within 1 year of residency) REQUIRED
  • Active KY license REQUIRED
  • Provider Must be local only mileage is reimbursed REQUIRED
