What are the responsibilities and job description for the RN - Intensive Community Mental Health Recovery Services - Chico position at VA Northern California Health Care System?
The Intensive Community Mental Health Recovery Services (ICMHRS) Registered Nurse will provide clinical community-based case management services to Veterans diagnosed with Severe Mental Illness (SMI), severe functional impairment, and high inpatient mental health unit utilization, in coordination with existing community and VA services.
This position is full time (M-F 0800-1630) and requires the RN to meet with the Veteran at their home or in the community.
The Registered Nurse is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on Mental Health (MH) specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the mental health continuum of care. Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team.
