The ideal Case Management Supervisor must be a licensed social worker or mental health professional in Minnesota. Experience in supervisory, managerial, or leadership positions is essential.

What will you do?
Our case management supervisors are responsible for providing clinical oversight for our adult mental health targeted case management program. In this position, you will allocate workloads to case managers and offer coaching and monitoring to ensure the team maintains a high standard of service delivery. You will encourage problem-solving and collaborative, person-centered services by facilitating weekly staff meetings and providing clinical consultation. Your duties will include conducting performance reviews, approving billable notes, functional assessments, and goal planning documents. You will also be tasked with making referrals, coordinating services, and evaluating the effectiveness of service delivery. Additionally, you will manage a small caseload, delivering case management services that address mental health, medical, financial, social, educational, vocational, housing, and legal needs, with ongoing assessment and evaluation of your responsibilities. Supervisors will also extend clinical supervision to non-licensed case management staff.
To be considered for this opportunity you will bring your skills and talent with a Master’s in Social Work, Marriage and Family Therapy, or Professional Counseling, be independently licensed (i.e. : LICSW, LMFT, LPCC), have a valid driver’s license, and adequate insurance coverage, and access to an automobile.
