Responsibilities
This position will be a hybrid position with two areas of concentration: housing resources and orientation for persons served in the ACT Case Management program.
This position will be assigned to the ACT team and will be responsible for collecting, organizing and disseminating information about community resources, housing options, educational opportunities, and entitlement programs to consumers and staff. The position will work with individuals exiting the Nursing Facilities for Mental Health and Osawatomie State Hospital. The position will include maintaining grant databases (HIMS), collecting needed demographic information, and other required information for the grant reports and applications. The Housing Resource Specialist is a liaison with community agencies to facilitate coordination of housing service provision.
A secondary function of this position will be responsible for coordination and access to Case Management on the ACT team, providing an overview of case management, facilitating preliminary onboarding and supporting persons served in transitioning to assigned case manager.
Job Requirements
This position requires: 1) a person with a bachelor’s degree in social/human service field; 2) a person with an Associates of Arts degree in Mental Health Technology with experience; 3) a Licensed Mental Health Technician with experience, or 4) an individual equally qualified by work experience or a combination of work experience and education, with one year of experience substituting for one year of education. Experience with persons with severe and persistent mental illness and/or chemical dependency is preferred. A valid Kansas driver’s license and access to reliable personal transportation and necessary insurance is required. In addition, the individual must be able to pass DMV, KBI, DCF Child Abuse, and DCF Adult Abuse Registry checks.

Salary
$21.62 – $24.67 – $28.24
Hours
8:00 am to 5:00 pm, some flexibility is required.
