Responsibilities
The essential role of this position is to orchestrate the integration of the State Hospital and Valeo staff in the discharge planning with Shawnee County residents. The liaison is responsible for monitoring the timely implementation of procedures related to outpatient treatment orders. The Liaison monitors the tracking of consumer’s county of responsibility and will negotiate transfer of residence consistent with consumer’s desire.
Job Requirements
This position requires three years of case management experience. This position requires: 1) a person with a Bachelor’s degree in social/human service field, 2) a person with an Associate of Arts degree in Mental Health Technology with experience, 3) a Licensed Mental Health Technician with experience, or 4) an individual equally qualified by work experience or a combination of work experience and education, with one year of experience substituting for one year of education. Prior case management, discharge planning, and experience with persons with severe and persistent mental illness and/or chemical dependency is required with a minimum of five years’ experience. Must demonstrate excellent communication, interpersonal, organizational and time management skills. A valid Kansas driver’s license and access to reliable personal transportation and necessary insurance is required.
Salary Range
$24.06 – $26.71 – $30.59
Hours
Mon – Fri, 8am – 5pm with flexibility required
