We are seeking a compassionate and organized Case Manager to provide addiction support, coordinate care, and help clients successfully transition back into daily life.
Monday- Friday (possible occasional weekends)

Case Manager
Responsible for providing addiction support, education, and care coordination to help clients transition back into daily life. Works with a multidisciplinary team to arrange post-treatment placement, connect clients with resources (housing, employment, legal aid, healthcare), and ensure continuity of care. Duties include developing discharge plans, coordinating referrals, maintaining documentation, and collaborating with families and providers.
Requirements:
- Bachelor's degree in Behavioral Health (preferred)
- 2 years of experience in substance abuse treatment
- Certified Recovery Specialist (preferred)
- Strong computer and communication skills
Job Type: Full-time
Pay: $22.81 - $27.47 per hour
