Job Title: Comprehensive Case Manager

The primary role of the Case Manager is to use strong human-centered, trauma-informed skills to empower individuals on their employment pathway, supporting their journey towards stability and success
Duties and Responsibilities:
- · Provide comprehensive case management and resource navigation to TEP participants on the employment pathway.
- Managing Salesforce as the primary data tracking and case management system.
- Maintain detailed case notes, track supportive service spending, and conduct barrier reduction assessments.
- Identify individual barriers to employment using the Bridge Assessment as guidance.
- Stay informed about community resources, conducting outreach to community partners for participant referrals.
- Offer compassionate support, including scheduling assistance and warm handoffs, during participant engagement with the community and resource connections.
- Transport clients to/from required offsite meetings such as chemical dependency intakes or court appointments.
- Issue supportive services to residents to aid in barrier reduction and employment pathway progression.
- Develop and execute housing stability plans, completing necessary documentation for Support Services.
- Monitor barrier reduction progress, preparing residents for enrollment into TEP (Transitional Employment Program).
- Guide participants to career services, facilitating connections to vocational training opportunities and/or workshops.
Minimum Qualifications:
- · Related college degree or a combination of training and relevant experience preferred.
- 2 years of experience working with high barriered individuals.
- Knowledge of adaptability in relating to individuals experiencing homelessness and/or reentry.
- Ability to work in a team environment or independently, constantly promoting a positive team spirit.
- Maintain a valid WA state driver's license and driving record.
- Strong computer literacy, including knowledge of Case Management System’s (Salesforce preferred), database management, and proficiency in Microsoft Office.
- Compassion-driven and interculturally competent.
- Extensive knowledge of practice through trauma-informed care.
- Ability to work fluidly with diverse populations facing multiple barriers.
- Excellent time management and organizational skills.
- Ability to operate in rapidly changing environments while ensuring efficient results.
