Job Summary
We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong understanding of patient needs, effective communication skills, and the ability to work independently. The Registered Nurse will be responsible for developing care plans, administering medications, and collaborating with healthcare professionals to deliver exceptional patient care.
Duties
- Conduct comprehensive assessments of patients' health status and needs.
- Develop and implement individualized care plans based on patient assessments.
- Administer medications and treatments as prescribed, including catheterization and use of neonatal ventilators when necessary.
- Monitor patients' progress and adjust care plans as needed.
- Maintain accurate medical records using eClinicalWorks and other documentation systems.
- Provide education and support to patients and their families regarding health management.
- Collaborate with interdisciplinary teams to ensure coordinated care for patients.
- Assist with memory care for patients requiring specialized attention.
- Participate in case management activities to optimize patient outcomes.
Experience
- Current Registered Nurse (RN) license in the state of practice.
- Previous experience in home health, assisted living, or travel nursing preferred.
- Knowledge of anatomy and medical terminology essential for effective patient care.
- Familiarity with electronic medical records systems such as eClinicalWorks is a plus.
- Strong interpersonal skills with the ability to build rapport with patients and families.
- Experience in memory care or working with diverse patient populations is advantageous.

Pay: $40.00 - $45.00 per hour
