Job Summary:
The Home Care Coordinator is responsible for the provision of home care to meet identified participant outcomes, and to achieve the goals as outlined in the participants Plan of Care
Duties/Responsibilities:
- Initiates the home assessment, planning, implementation and evaluation of the home care portion of the IDT plan.
- Coordinates all home care needs according to established policies and procedures.
- Contacts and arranges contracted services to meet participant needs as necessary and approved by IDT and in accordance with the care plan and all regulations.
- Uses proper techniques, procedures, and safety precautions to meet individual needs of participants.
- During the home assessment process, utilizes UCAT III, observation and communication to determine the participants needs with regard to IDLs and ADLs, home safety, community safety, nutrition, maintenance, house care needs.
- Reports to IDT and records in EMR any change in condition or behavior of the participant.
- Follows IDT care plan, and observes established policy and procedures to assure proper treatment and care of participant.
- Manages the schedules, behaviors and education of the CNAs and CHHAs employed by Valir. Ensures all care is provided to appropriately meet patient needs as identified in care plan as well as in accordance with
- Recognizes change in participant condition or behavior and reports same to IDT and documents in EHR.
- Assists in orientation of home care attendants and contracted home care providers.
- Participates in participant and family teaching and instruction.
- Able to perform in an accurate and precise manner in crises and emergency situations when time is of the essence
- Establishes good rapport with participants, families, and visitors, respecting participants rights to privacy and confidentiality of information.
- Accurately communicates in writing through utilization of appropriate charting method and proper transcription of medical orders.
- Provides direction to participants, families, and visitors in a courteous, precise manner with a positive tone.
- Accurately communicates participants information to IDT.
- Independently performs assigned duties, willingly cooperates with IDT in meetings and other committees as deemed necessary by the Governing Board and/or Director.
- Recognizes and responds to priorities in the participant care process. Provides proper notification for absence and tardiness, and is punctual at start of shift.
- Assist in maintaining equipment assigned to patient care, and reports malfunctioning of equipment to appropriate personnel.
- Maintains accurate and timely record keeping
- Maintains a clean and safe environment for participants and other home care staff. Demonstrates understanding of emergency procedures, fire drills, evacuation procedures, and internal/external disaster plans and procedures.
- Any and all other duties and responsibilities as assigned.
Education, Licenses, Certifications and Experience:
- Current Long Term Care Aide and Current Home Health Aide Certifications
- Current CPR Certification
- Valid Driver's License & reliable transportation
- Either one year working with the frail and elderly population or approved training.
