Responsibilities
- Provide direct patient care under the supervision of registered nurses and physicians.
- Assist with case management by coordinating patient care plans and ensuring continuity of care.
- Administer medications and treatments as prescribed, including sterile processing when necessary.
- Conduct triage assessments to prioritize patient needs in a timely manner.
- Monitor patients' vital signs and report any changes or concerns to the healthcare team.
- Educate patients and their families about health conditions, treatment plans, and wellness strategies.
- Collaborate with healthcare professionals in internal medicine and long-term care settings.
- Participate in training programs and contribute to the teaching hospital environment by mentoring nursing students or new staff.
- Maintain accurate patient records using electronic health record systems such as Athenahealth.
Qualifications
- Valid Licensed Practical Nurse (LPN) license in the state of practice.
- Experience in case management, triage, or internal medicine preferred.
- Familiarity with sterile processing techniques is a plus.
- Knowledge of dermal fillers is advantageous but not required.
- Strong interpersonal skills with an emphasis on patient-centered care.
- Ability to work collaboratively within a healthcare team while demonstrating leadership qualities.
