What are the responsibilities and job description for the ACT PSH Case Manager position at Valley Community Services Board?
Valley Community Services Board is seeking a dynamic individual to join our Assertive Community Treatment Team.

Essential Functions:
- Work with a population of adults who may be dually diagnosed with chronic mental illness and substance use issue.
- Provides intensive case management, rehabilitation, and supportive services to individuals experiencing serious mental illness.
- Assist the formerly homeless individuals maintain their housing within the permanent housing program. Services are provided in accordance with the ACT model of treatment and Housing First Principles. Services are provided in multiple settings, including in the home and in the community.
- Provides emergency/crisis intervention and coordinates for hospital prescreening.
- Assists in the provision of medical services, such as delivering and monitoring medications.
- Provides clinical and program consultation to individuals, organizations, agencies, groups, etc. that are involved with Valley Community Services Board consumers.
Minimum Qualifications:
- Bachelor or Master's Degree in human services field and one year direct clinical experience with mentally ill population; valid VA driver's license with an acceptable driving record. Must be QMHP or QMHP-T eligible.
- Physically able to climb stairs and assist with moving small to moderate sized items; Is able to carry out job responsibilities within the existing setup of the ACT office.
- Can work outside the office setting in the community and can tolerate heat and cold as well as inclement weather conditions.
- Can work independently in the environment.
- QMHP or QMHP-T required.
- Virginia driver’s license in good standing is required.
The ACT PSH position is full-time and requires working a schedule rotation of evenings, weekends, and holidays. There are limited on-call responsibilities.
