What are the responsibilities and job description for the School Based Child MH Case Manager position at Valley Community Services Board?
VCSB is looking for a School-Based Child Mental Health Case Manager to join our team! The School-Based Child Mental Health Case Manager will provide intensive case management services to children and adolescents with seriously emotionally disturbed (SED)/at-risk behavior, in a community or school setting. The child case manager works collaboratively with child and family clinicians, school-based outpatient clinicians, and school staff/administration.

Summary of Essential Functions
- Provides activities and case management services to children and adolescents with at-risk/SED behavior.
- Collaborates with children, adolescents, their families, and school administration to facilitate adjustment at home, in school, and in the community.
- Helps children, adolescents, and their families navigate crisis stabilization and emergency intervention services as needed.
- Carries a cell phone to provide crisis stabilization and emergency intervention services as needed.
- Develops and completes Individual Service Plan (ISP) reviews on caseload in accordance with agency standards and Medicaid criteria.
