Nature of Work:
To provide skill development training and supports to individuals with mental health/substance abuse diagnoses. This involves assessing individuals, planning and implementing programs, and documenting services provided.

Essential Duties:
- Encourage good personal hygiene and health by modeling and motivating individuals to maintain their personal care and health.
- Document services provided, including direct services progress notes, transportation notes, and behavior tracking forms, in a timely and accurate manner.
- Perform all predetermined services as per doctor's orders.
- Maintain the safety, health, and well-being of individuals on or off site.
- Implement treatment/behavior plans in compliance with Valley and State guidelines.
- Engage individuals in achieving their daily goals and objectives through active treatment.
- Ensure personal care, changes in health status, and hygiene needs are met for individuals.
- Complete all job duties as assigned within the specified timeframe or shift as scheduled, in compliance with Valley Policies and Procedures, State and Federal regulations.
Qualifications:
- Ability to perform essential duties.
- High School Graduate or equivalent.
- Valid Driver's License.
- Ability to obtain and maintain CPR, First Aid, Mandt certifications, and other required trainings.
- Ability to comply with Client Rights and Valley and division departmental safety procedures.
- Ability to read, write, and speak the English language.
