ESSENTIAL DUTIES:

  • As a role model, group leader, and motivator, Community Skills Trainers will facilitate and assist consumers in achieving goals of wellness and daily living skills (i.e. grooming, hygiene, nutrition, cooking, cleaning, taking medications appropriately, etc.).
  • Documentation as required by Valley HealthCare System, Medicaid, OSHA, and other licensing agencies. Employee ensures that documentation (such as Direct Services Progress Notes, Client Transportation Progress Notes, etc.) are completed accurately and timely and will reflect services the client(s) has received services as appropriate.
  • Perform all-predetermined services as per Individual Treatment Plan.
  • Complete Electronic Medical Record (EMR) Documentation during each daily episode of care, notating, clinically, clients progression/regression in each of the clients individualized treatment goals identified in Master Treatment Plan
  • May attend client Interdisciplinary Team meetings and participate in Treatment Plan development
  • Assure that the safety, health and well-being of consumers are always maintained on or off site.
  • Implement treatment/behavior/ plans in compliance with Valley HealthCare Systems and State guidelines.
  • Completes and submits program specific documentation in a timely and accurate manner.
  • Monitor, maintain and use appropriately, all equipment and supplies.
  • Transportation of consumers to appointments, activities in the community, and to/from Program site, using a company vehicle or employees personal vehicle, on rare occasions.
  • Support and help in the training of new employees.
  • Attends in-service trainings, mandated meetings and completes Essential Learning trainings as assigned.
  • Employee will engage consumers in achieving their daily goals and objectives through active treatment.
  • OTHER DUTIES AS ASSIGNED.

NATURE OF WORK: To provide ongoing regularly scheduled pseudo-therapeutic support, instruction, and activities to consumers with mental illness, intellectual/development disabilities, and/or substance use disorder diagnoses for the purpose of increasing consumer wellness recovery and daily living skills. This involves consistent consumer assessment, program planning and implementation. The Community Skills Trainers will work in both a Day Treatment group setting, as well as out in the community with consumers, including their home.

MINIMUM QUALIFICATIONS:

  • Ability to perform Essential Duties as outlined below.
  • High School Diploma or equivalent.
  • Valid Drivers License
  • Ability to obtain and maintain CPR, First Aid, and Mandt Certifications.
  • Ability to comply with Consumers Rights.
  • Ability to comply with Valleys and Division departmental safety procedures.
  • Ability to read, write and speak the English language.
  • Basic Computer Skills (Typing, Microsoft Word)
  • Ability to document services in an Electronic Medical Record (EMR)

PREFERRED QUALIFICATIONS:

  • BA/BS in Human Service Field or working towards this.
  • Experience working with individuals with Intellectual/ Developmental Disabilities, Mental Illness and/or Substance Use Disorders
  • Ability to work flexible hours on occasion
  • Experience as part of a treatment team
