Job Description:
The Director of Case Management is responsible for the assessment, planning, coordination, development and evaluation of the Case Management program. This includes the functions of case management, utilization management, discharge planning and social services. Accountable for the assessment and monitoring of the care and services rendered by the department staff. Also responsible for staffing levels, competency and educational needs of the case management staff to ensure effectiveness of the coordination and control of resources. Provides consultation and education to facility leadership, clinical and financial personnel and medical staff members related to case management, utilization and discharge planning practices. Ensures compliance with local, state, federal and private agencies as relates to Case Management and Utilization Management. Provides
leadership in the fulfillment of UHS/VHS driven initiatives and expectations.
Education:
Licensed Social Worker, MSW required OR Graduate of an accredited school of Nursing, BSN required; Master's preferred.
Experience:
A Minimum of five (5) years clinical experience required and a minimum of two (2) years progressive management experience required. Must have knowledge of social and physical factors that affect functional status at discharge, and knowledge of community resources to meet post discharge clinical and social needs.
Technical Skills:
Has a current Social Worker OR Registered Nurse license in the State of Nevada.
License/Certification:
Must be able to demonstrate the knowledge and skills necessary to provide care/service appropriate to the age of the patient.
Other:
Full knowledge of pertinent regulatory agencies and the policies, codes, standards, and regulations for a healthcare environment

Salary : $101,501 - $134,488
