The LPN Case Manager will assist the RN Case Managers in their daily role and processes to organize and facilitate thorough discharge plans for the patients. The LPN Case Manager will be responsible for completing any clinical assessment for patients requiring post-acute services under State Medicaid programs including, but not limited to, Virginia, West Virginia, and Maryland. The LPN Case Manager will be responsible for completing pre-screens, admissions, and insurance authorizations for Valley Health's skilled nursing facility and transitional care beds.
Extra duties include, but are not limited:
Assisting Case Management staff with completion of Level 2 (MIMR) assessment.
Interviewing patients, family members, and/or caretakers as needed to complete assessments.
Maintaining open communication with care management team regarding approval/denial of clinical assessments.
Maintaining proficiency in electronic submission programs and awareness of current policies/trends related to clinical assessments for Medicaid services.
Assists with the arrangement and follow-up with community resources. LPN Case Manager will assist in providing a seamless transition utilizing the entire continuum of care and helping to facilitate patients movement to the next level of care.
Education
Graduate of an accredited school of nursing required
Experience
Two years bedside experience required
Certification & Licensure
Licensed as a Licensed Practical Nurse in the State of Virginia required
BLS Certification (Basic Life Support) - American Heart ˜Healthcare Provider' (HCP) - AHA approved is required. * New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.
Qualifications
Competent to successfully interact with patients according to their special needs which may apply based on the age of the patient served.
