Job Summary
Valley Health is seeking a skilled Home Health Physical Therapist to join our team. As a Home Health Physical Therapist, you will provide direct patient care in a home environment as part of a multidisciplinary team.
Responsibilities
- Evaluate patients and design individualized plans of care.
- Provide direct, one-on-one patient care and supervise care provided by assistants.
- Work under the direction of a clinical team manager with support from a rehab clinical coordinator.
Requirements
- Bachelors, master's or doctoral degree in Physical Therapy is required.
- A completed CI credentialing course, home study adult learning course, or equivalent, is required.
- Minimum two years of clinical experience (4,160 hours) is required.
- Physical Therapy licensed or eligible in the state of assignment (Virginia or West Virginia) is required.
- BLS Certification (Basic Life Support) - American Heart ˜Healthcare Provider' (HCP) - AHA approved is required.
- Valid driver's license and reliable transportation is required.
