Summary: This is a part-time on-site role for an Outreach Worker at Valley Healthcare System, Inc. The Outreach Worker will be responsible for day-to-day tasks such as conducting community outreach, providing case management, and utilizing their interpersonal and communication skills. The role requires close collaboration with the community and team members.
Location: Columbus, Talbotton, Fortson; some travel required.
Salary: $22.96/hour
Duties and Responsibilities:
- Distribute outreach materials to patients to build coverage option awareness.
- Provide orientation to new patients concerning the purpose, conditions, and structure of programs that are available to assist with their needs.
- Will assist in community outreach and engagement, and support in connecting community members with resources and services.
- Assist with addressing vaccine hesitancy within these communities and other challenges including difficulty navigating processes.
- Serves as a resource in the development and implementation of outreach communication.
- Provide support to front-end staff to properly collect data for UDS.
- Serve as a patient advocate in obtaining the cooperation of outside service providers, family members, school officials, etc. when necessary.
- Research and assist in developing plans for expanding the VHcS presence.
- Serve as an ambassador of the mission of VHcS in the interaction with current and potential partners.
- Participate in off- site activities to include workshops, health fairs, and community presentations.
- Will make appointments where needed for patients to come in to receive outreach services.
- Perform any other duties asked by supervisor when needed.
- May provide enrollment assistance (including, but not limited to, completing coverage applications, gathering required documentation, and troubleshooting the enrollment process) for uninsured and underinsured patients to access subsidized, low-cost, and free health insurance programs through the Health Insurance Marketplace, Medicare, Medicaid, and the Children’s Health Insurance Program (CHIP).
Evaluation: The evaluation of work performance, including specific duties, responsibilities, demeanor, and work ethic, will be ongoing and implemented by the CEO per center policies.
Requirements:Minimum Qualifications:
- High school diploma or GED
- Two years of experience working with a diverse population.
- Bilingual Spanish preferred
- Must possess a valid Georgia’s driver’s license.
- Preferred be ACA/Navigator certified.
Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift up to 15 pounds at times.

Security (HIPAA): Every employee is required to protect their computer log-on and password. You must secure your work area prior to leaving it unattended for an extended period of time. You are required to log-off of your computer and secure your work area prior to leaving at the end of the day. If you have a responsibility for closing or locking files, securing equipment, or locking the building and you do not comply with these requirements, you will be subject to reprimand and/or termination based upon the infraction. We are required by law to protect patient privacy, identifiable patient information, and all properties containing such information.
Physical Requirements: Varied activities include standing, walking, reaching, bending and lifting. Employee must possess full range of body motion including the ability to assist or lift patients when necessary; occasional weight bearing to exceed 50 pounds, use of portable stretchers/wheelchairs encouraged. Employee must possess hand-eye coordination with normal range of vision and hearing abilities. This position frequently requires working under stressful conditions and frequently requires irregular work hours.
Environmental Working Conditions: This position includes both administrative and medical responsibilities. Employee is frequently exposed to communicable diseases, toxic substances, medical cleaning and processing solutions which my included ionizing radiation plus other elements common in a clinical environment.
