Summary
As a Licensed Practical Nurse at Valley Hospice Inc., you will play a vital role in delivering high-quality hospice care to patients in need. This position is essential in ensuring that our patients receive the compassionate support they deserve during their final stages of life.
Responsibilities
Essential Functions/Performance Factors
- Provides services in accordance with organizational policies and procedures and regulations that define scope of practice. Primary responsibility is to provide crisis care nursing.
- Complete pain assessment thoroughly with each visit.
- Identify other symptoms and treat appropriately.
- Provides skilled nursing care to patients on the Care Centers under the direction of the Registered Nurse.
- Using the NAR (Nursing Assessment Record) as a tool or, assess patient’s physical condition with each visit & document findings.
- Complete pain assessment thoroughly with each visit.

- Provides skilled nursing care to patients on the Home and Facility teams when required to provide consistent optimal patient care according to staffing needs in accordance with organizational policies and procedures.
- Provides skilled nursing care in the on-call setting under the RN’s supervision, as required for staffing needs in specific service territory. Implement and evaluate nursing care through systematic assessment of patient family needs, in accordance with organizational policies and procedures.
- Participates in assessing, implementing and evaluating the nursing care plan according to the IDT Care Plan.
- Implements the care plan related to identified patient/family needs.
- Evaluates the effectiveness of nursing interventions, communicate findings to RN and make appropriate changes to the care plan.
- Attends team meetings, as appropriate, and participate in the development of the plan of care.
- Reinforces the RN’s education to patient’s/families about palliative care, infection control and issues related to death and dying.
- Assess patient/family educational needs with each visit and respond with appropriate teaching under the direction of the RN.
- Evaluate patient/family response to teaching and change approach, as necessary, to meet needs.
- Use hospice teaching tools appropriately.
- Documents care provided in an accurate, complete and timely manner.
- Complete documentation of each nursing visit within 24 hours of visit.
- Complete EMR documentation accurately.
- Write nursing notes that reflect the patient’s terminal illness according to NHO guidelines, Local Medical Review Policies and the care plan.
- Sign all entries with full signature & credentials.
- Provide care/medications in accordance with a physician’s order.
- Chart to reflect the nurse’s role as case manager.
- Document change in status accurately and complete this documentation within 24 hours.
Qualifications
- Current LPN licensure in the state of practice (OH and WV).
- Experience in hospice or long-term care settings preferred.
- Compassionate approach to patient care with excellent communication skills.
- Ability to work collaboratively within a hospice team.
