Resource Specialists help seniors maintain their independence by providing information about Valley Senior Services nutrition and transportation programs. They coordinate and determine eligibility for the Meals on Wheels program. They also provide information and referrals to other in-home services and financial assistance programs.
ESSENTIAL FUNCTIONS:
Evaluation of this position is based primarily on performance of the following essential functions, which include, but are not limited to:
- Engage with and educate the community on services offered through Valley Senior Services
- Help clients identify current and future needs
- Explore various options of support
- Provide information regarding community resources
- Work with community partners and coordinate service referrals
- Protect client identity and confidentiality
- Point of contact for home delivered meal and resource services
- Initial determination and annual reassessment of client eligibility through database documentation
- Provide support with planning and coordination of Meals on Wheels delivery program
- Deliver Meals on Wheels
- Assist clients in accessing programs and services available through federal, state, and local agencies
- Support the organization as needed
OTHER FUNCTIONS:
Evaluation of this position may also be based on performance of the following other functions, which include, but are not limited to:
- Keeps direct supervisor promptly informed of key/significant issues or concerns.
- Carries out duties and responsibilities in a safe and efficient manner to maintain safe work environment and surrounding area for oneself, co-workers and public. Responds to hazardous situations and/or potentially unsafe conditions by taking corrective action as capable and promptly notifying supervisory personnel.
- Updates and enhances own knowledge by involvement in continuing education for professional growth, i.e., attends relevant conferences, seminars, in-service trainings, and certification programs.
Experience and Skills:
Education/Experience:
- Bachelor’s Degree in Social Work or human services field or equivalent work experience preferred
- Previous experience working with seniors preferred
Credentials/License/Certifications
- Must possess a valid driver’s license and access to an automobile
