We're looking for a full time Behavioral Health Advocate to join our Women's Health team!
Bilingual English/Spanish required!!
As the Behavioral Health Advocate, you will
- Under direction from the Behavioral Health Supervisor and Women’s Health Director, provide care coordination services to patients who are most at risk for health deterioration, Social Determinates of Health, sentinel events and/or poor outcomes.
- Assist the Women’s Health (WH) team and the Behavioral Health Provider (BHP) in maintaining a central role in the coordination and management of care of vulnerable patients and that patients receive comprehensive care across multiple health settings and providers; to ensure continuity and coordination of care across transitions of care for all patients, with special attention to patients with complex medical, behavioral and social needs.
QUALIFICATIONS:
Education:
- Associate or bachelor's Degree in related field is preferred.
- Significant experience in the behavioral health and/or substance abuse field will be considered, in place of a degree.
- Current BLS through the American Heart Association (AHA) or obtained within 3 months of hire.
Skills:
- Bilingual (English/Spanish) preferred.
- Ability to work independently.
- Ability to understand and present verbal instructions and to exchange verbal information.
- Ability to establish and maintain effective, courteous working relationships with patients/families, co-workers and others.
- Ability to work under pressure in a fast-paced environment.
- Ability to prioritize and perform multiple tasks.
- Ability to work in Windows based computer environment.
- Ability to work in professional manner.
Experience:
- 1 year of medical or behavioral health experience, required.
- Preferred: Experience working with Substance Abuse issues.
CAC I or higher preferred or willing to obtain.
SALARY RANGE
- $20.00 to $28.87/hour
- Entry salary dependent upon education, skill set, and experience.
