Position Overview
We are seeking a Behavioral Health Provider for our:
Guadalupe Health Center located in Antonito, Colorado.
In this role, you will be part of an integrated, primary healthcare network using a Team-Based Care (TBC) approach. Your focus will be on holistic, preventive, therapeutic, and educational services for patients of all ages while promoting patient and family-centered care.
Job Responsibilities
Clinical Care: Perform assessments, identify problems, justify medical necessity (using DSM-5 criteria), and deliver evidence-based treatments, crisis intervention, and individual/family/group therapy.
Collaboration: Work within the Primary Care Behavioral Health (PCBH) model to develop and implement treatment plans through consultation, brief assessments, and psychoeducation.
Documentation: Maintain accurate and up-to-date patient records in our EMR system (NextGen), including intake documentation and treatment plans.
Compliance: Uphold HIPAA regulations and patient confidentiality at all times.
Qualifications and Education
Educational Background:
Actively pursuing a Masters or Doctoral degree in psychology, counseling, or clinical social work with at least one year of completed schooling is acceptable.
A minimum of a Masters degree (MA) in a related field is required if not currently enrolled in a graduate program.
Licensure:
Candidates may hold a current license with the Colorado State Board as any of the following:
Licensed Clinical Psychologist (Ph.D., PsyD)
Licensed Clinical Social Worker (LCSW)
Licensed Professional Counselor (LPC) (preferred)
Licensed Marriage and Family Therapist (LMFT)
Additional Requirements:
Bilingual skills in English/Spanish are desirable but not required.
Must successfully complete drug testing and background screening.
