We are currently seeking Medicare Wellness Pod Support based out of our Valley-Wide Canon City Clinic or La Junta Clinic to join our team. In this role, you will ensure seamless care by assisting patients, staff, and care teams through phone, patient portal, and in-person interactions. Youll foster a culture of patient-centered care, enhancing experiences, improving operations, and promoting excellence in quality and safety.
Key Responsibilities:
Patient Assistance (Phone & In-Person)
- Respond to patient inquiries via phone and in-person, ensuring efficient and effective communication.
- Handle urgent patient needs within specific timeframes.
- Manage patient portal requests and schedule appointments accordingly.
- Facilitate follow-up appointments before patient departure from the clinic.
- Coordinate with Nursing Coordinator and clinical staff to triage patient symptoms in urgent situations.
- Ensure timely documentation and communication of patient messages to medical personnel.
Authorization & Scheduling Support
- Process authorizations for imaging and diagnostic procedures.
- Manage incoming referrals, maintain reports, and communicate with ordering providers.
- Schedule imaging and diagnostic appointments with performing entities and inform patients.
- Document authorizations and appointment details accurately in NextGen.
Qualifications & Skills:
- High school diploma or GED required.
- Basic Life Support (BLS) certification/training will be provided upon employment.
- Strong ability to prioritize work and manage time effectively.
- Proficiency in Microsoft Word, Excel, and Outlook.
- Professional communication skills, both in person and over the phone.
- Ability to collaborate effectively with clinical teams.
