We are seeking a compassionate and skilled Registered Nurse to join our Nurse-Family Partnership team. This position is responsible for providing comprehensive, evidence-based support to first-time mothers and their families through regular home visits. The RN will help women achieve healthier pregnancies, improve child health and development, and reach personal goals.
Key Responsibilities:
- Assess physical, emotional, social, and environmental needs of women and families.
- Collaborate with clients to set realistic goals for health and life development.
- Provide education and counseling on prenatal/postpartum care, nutrition, parenting, well-child care, family planning, and more.
- Monitor progress toward goals and revise care plans as needed.
- Establish strong therapeutic relationships to support problem-solving and personal growth.
- Promote family involvement in pregnancy, birth, and early child care.
- Connect clients with appropriate community resources.
- Collaborate with other healthcare and service professionals to ensure coordinated care.
- Document nursing activities within 24 hours of visits; submit paperwork weekly.
- Provide feedback on protocols and actively participate in team development.
- Attend all required program trainings and contribute to outreach efforts.
- Maintain full caseload per Nurse-Family Partnership protocols.
- Complete home visits per program requirements, maintaining appropriate duration and content focus.
- Ensure timely contact and follow-up with referrals.
- Meet weekly with supervisor and participate in joint visits as needed.
- As a licensed nurse, perform additional office duties within scope and maintain credentialing and competencies as required.
Education & Experience/Skills:
Education: Baccalaureate Degree in Nursing and Current Colorado RN License (Required)
Experience/Skills:
- Minimum of 2 years' experience working with childbearing women and young children preferred
- Community health experience preferred
- Bilingual skills desirable
- Strong communication skills and cultural sensitivity
- Ability to build trusting relationships with families from diverse backgrounds
- Comfortable with home visits and maintaining accurate documentation
