Key Responsibilities
Patient Advocacy & Support:
- Assist patients in overcoming healthcare, financial, and social barriers.
- Support enrollment in programs such as Medicaid, Sliding Fee Discount, and transportation assistance.
- Maintain confidentiality and ensure patients understand their rights and options.
Comprehensive Patient Assessments:
- Conduct in-depth assessments of new and referred patients, addressing both clinical and psychosocial needs.
- Provide guidance on medical, dental, and behavioral health recommendations to improve patient health outcomes.
- Act as a liaison between patients and care teams, ensuring a smooth care coordination process.
Navigation & Referral Coordination:
- Manage external and internal referrals, ensuring timely follow-up and closure of care loops.
- Assist patients in navigating insurance authorizations, scheduling imaging and diagnostics, and appealing denied claims.
- Track, document, and monitor treatment plans and referral statuses.
Collaboration & Communication:
- Work closely with interdisciplinary healthcare teams to optimize patient care.
- Utilize motivational interviewing techniques to engage patients in their health journey.
- Document all navigation activities in the Electronic Health Record (EHR) system.
Community Engagement & Quality Improvement:
- Stay informed about internal and community resources to effectively connect patients with appropriate services.
- Support quality improvement initiatives by tracking preventive screenings and patient outreach efforts.
- Participate in population health management efforts for targeted patient groups.
Qualifications & Skills
Experience:
- At least one year of experience in customer service, patient support, case management, or healthcare coordination.
Education & Certifications:
- High school diploma required; a bachelor’s degree in healthcare administration, human services, or a related field is preferred.
- Bilingual Spanish-speaking skills are highly desirable.
